What is the Pelvic Floor and What Role Does it Play?

The pelvic floor is a crucial part of our body, consisting of muscles, nerves, ligaments, and fascia that form a hammock-like structure at the base of the pelvis. Its role spans far beyond s support; it’s integral to maintaining control over our bowel and bladder, supporting our pelvic organs, contributing to sexual function, and stabilizing our core.

As you can imagine, pregnancy results in an increased demand on these muscles as they continue to support our organs as well as the growing baby for the duration or pregnancy.

During childbirth, the muscles that make up the pelvic floor play a unique role—their job is to relax in order to allow the baby to pass through the vaginal canal. In fact, the pelvic floor muscles need to stretch 1.5-3 times their length as the baby passes through. Pregnancy and delivery can often lead to pelvic floor muscle tightness or weakness postpartum.

Individuals who undergo a C-section delivery may also experience pelvic floor dysfunction as their pelvic floor supported the growing baby for the duration of pregnancy and their abdominal wall is also compromised after surgery.

A C-section involves an incision through 7 layers of tissue including the abdominal muscles. As the abdominal wall makes up part of our core system along with the pelvic floor, a C-section may have indirect effects on the function of the pelvic floor.

Common Pelvic Health Concerns Postpartum

Several concerns may arise after childbirth, including:

  • Urinary or fecal incontinence
  • Symptoms of pelvic organ prolapse
  • Pelvic or low back pain
  • Sexual dysfunction such as pain during intercourse or difficulty having penetrative intercourse
  • Decreased core stability

What is a “Normal” Recovery Time from Childbirth?

It is normal for pelvic health symptoms to persist for weeks to months postpartum. A pelvic health physiotherapist can help guide you on your individual journey of recovery.  If these symptoms linger or worsen, it is essential to seek guidance from a healthcare provider specializing in pelvic health or postnatal physiotherapy. While these issues are common, they shouldn’t be considered a normal part of life.

How Can Postnatal Physiotherapy Help You?

A pelvic health physiotherapist can provide personalized care to address postpartum concerns. This may include the following:

  • A comprehensive assessment, which may include internal examinations if indicated.
  • Education on how to properly contract and relax the pelvic floor muscles.
  • Developing an individualized exercise program including pelvic floor muscle lengthening, and or strengthening to improve core stability.
  • Addressing lifestyle factors such as constipation and stress management to support overall pelvic health.

When Should I See a Pelvic Health Physiotherapist?

It’s advisable to seek guidance from a pelvic health physiotherapist whenever you feel the need to address any of the issues you are experiencing.

Internal assessments are generally safe after six weeks postpartum, provided you’ve been cleared by your OB/GYN or midwife. However, it is never too late or too far after your childbirth experience to take steps towards improving your pelvic health.
